This plan is (a reasonable/ an affordable) way to to help budget yearly regular dental needs for you or your family.

• 2 Basic Dental Cleanings- (Excluding Scaling and Root Planning- “Deep Cleaning”)

• 1 Periodic Exam during Cleaning
• 1 Emergency Exam (If needed)
• X-Rays (Bitewing X-rays 1x yearly)
• X-Rays (Two Periapical X-ray)
• 1 Oral Cancer Screening and 1 Fluoride Treatment
• 5-15% courtesy discount on all dental treatment provided at Graham Family Dental within the 12 months of enrollment. (Offer excludes Lab Fees)
• Save 10% on Orthodontic Treatment

Details:

Premiums are yearly and benefits are in effect for one year from the date of paid enrollment. Your annual plan benefits begin the moment your annual fee is paid in full. All benefits must be used within the premium year (there is no carry-forward credit). The initial Fee is nonrefundable.

An additional fee may be charged for any missed, canceled, or broken appointment without 24 hours prior notice.

*To receive the 15% discount on other dental procedures in the office, the total balance must be paid in full at the time of service, unless a written agreement is completed prior to treatment.

Travis Graham Wyatt DDS reserves the right to refuse treatment and/or terminate the patient’s participation in this Plan with 30 days written notice if the patient’s account becomes delinquent or the patient is non-compliant. This Plan may be modified, amended, or canceled at any time with 30 days written prior notice and may be subject to other terms and conditions. Plan participants are responsible for scheduling their periodic treatments, and services not used at the end of each year’s membership period are not carried over to the following year. Discounts apply only to services performed by Travis Graham-Wyatt and does not include treatment or services provided elsewhere even if you are referred to a specialist.

Terms and Conditions, “The Fine Print”

This is a dental discount plan and NOT dental insurance.

It CANNOT be combined with any other dental insurance, it is good only at Graham Family Dental. Therefore, if you are referred to a specialist, they will NOT offer this discount.

Should there be dental treatment needed following any type of injury where a lawsuit and therefore outside medical, auto or car, disability, or workman’s comp type insurances are involved, this discounted plan cannot be used.

This plan is Non-Refundable and Non-Transferable

Any “unused” benefits cannot be refunded, credited, or carried over to the next annual period. Plan is subject to change yearly.

All payments are due AT TIME OF SERVICE to receive the discount. Any services received that are not paid for at the time of service will be billed at the usual, higher fee. If you choose to pay for treatment by using Care Credit, Credit cards, or another healthcare financing firm, the restorative treatment discount is reduced to 5% due to merchant fees involved. This offer cannot be combined with any other offers. Failure to comply with the terms of the Plan may result in termination of the Plan and forfeiture of any fee paid by the patient or third party.

Plan fees are valid only when paid at the time of enrollment.
